What is Lenzetto spray?

Lenzetto is a Hormone Replacement Therapy (HRT) that contains the female hormone oestrogen. It is intended for use in postmenopausal women who have not had a natural period for at least 6 months. Lenzetto can also be used by women who have undergone surgery to remove their ovaries, leading to an immediate onset of menopause. Lenzetto is administered as a spray solution containing estradiol. When applied to the skin as directed, the estradiol is absorbed through the skin and into the bloodstream.

What is Lenzetto spray used for?

Lenzetto is a Hormone Replacement Therapy (HRT) containing the female hormone oestrogen. It is designed for postmenopausal women who have not had a natural period for at least 6 months. Lenzetto is also suitable for women who have undergone surgery to remove their ovaries, which triggers immediate menopause.

Lenzetto is administered as a spray solution containing small amounts of estradiol. When applied to the skin as directed, the estradiol is absorbed through the skin and into the bloodstream.

Uses of Lenzetto:

  • Relief of Menopausal Symptoms: Lenzetto is used to alleviate symptoms associated with menopause, such as hot flushes, which occur when the body's oestrogen levels decrease. It is prescribed when these symptoms significantly impact daily life.
  • Treatment of Oestrogen Deficiency: Lenzetto treats symptoms of oestrogen deficiency after menopause, including hot flushes, sleep disturbances, irritability, and vaginal dryness.

The experience of using Lenzetto in women over 65 years old is limited, and it is important to note that Lenzetto is not a contraceptive.

How does Lenzetto spray work?

Lenzetto spray works by delivering a form of the female hormone oestrogen, called estradiol, directly into your bloodstream through the skin. As a woman goes through menopause, her body produces less oestrogen, which can lead to symptoms such as hot flushes, night sweats, and vaginal dryness.

When Lenzetto spray is applied to the skin, the estradiol is absorbed and helps to replenish the body's oestrogen levels. This increase in oestrogen alleviates menopausal symptoms by restoring hormonal balance, thereby improving comfort and quality of life for postmenopausal women.

How do you use Lenzetto spray?

Always use this medicine exactly as your doctor has instructed. If you are unsure, consult your doctor or pharmacist.

Your doctor will aim to prescribe the lowest effective dose to manage your symptoms for the shortest time necessary. During treatment, your doctor may adjust the dose according to your individual needs. If you think the dose is too strong or too weak, speak to your doctor.

If you have not had a hysterectomy (surgery to remove your womb), your doctor will prescribe additional tablets containing a hormone called progestogen to balance the effects of oestrogen on the lining of your womb. Your doctor will explain how to take these tablets. You may experience withdrawal bleeding at the end of the progestogen treatment period.

If You Need Surgery

If you are scheduled for surgery, inform your surgeon that you are using Lenzetto. You may need to stop using Lenzetto about 4 to 6 weeks before the operation to reduce the risk of blood clots. Ask your doctor when you can resume using Lenzetto.

Where to Apply Lenzetto

Apply the spray to clean, dry, and healthy skin on the inner forearm. If this is not possible, apply it to the inner thigh. Do not apply Lenzetto to your breasts or near the breast area.

How to Apply Lenzetto

Before using a new applicator for the first time, prime the pump by spraying three times with the cover on:

  • Hold the container upright. With the cover on, press the button down three times using your thumb or index finger. The medicine is now ready to use.
  • Note: You do not need to prepare the applicator before each dose—only before starting a new container. If you miss one or more doses, prime the applicator as instructed.

Ensure the skin where you want to spray the medicine is clean, dry, and healthy.

Applying Your Daily Dose

  • Remove the plastic cover, hold the container upright, and place the plastic cone flat against the skin.
  • You may need to adjust the cone so it is flat against your skin with no gaps.
  • Press the actuator button down fully and hold it down before releasing.
  • If more than one spray is required, move the cone along your arm to a new spot and press the button down again. If necessary, you can use your other inner forearm or the inner surface of your thigh.
  • After applying the spray, replace the cover on the container.

Let the spray dry for at least 2 minutes before dressing and wait at least 60 minutes before bathing or washing. If you accidentally get Lenzetto on another area of your skin, wash that area immediately with soap and water. Do not massage or rub Lenzetto into the skin.

How Much Lenzetto to Use

Your doctor will likely start you on the lowest dose (one spray per day). If necessary, your doctor may increase the dose to two sprays per day, with a maximum of three sprays per day.

Frequency of Use

Apply the total number of sprays prescribed by your doctor at the same time each day.

Duration of Use

Discuss with your doctor every 3-6 months how long you should continue using Lenzetto. Use Lenzetto only for as long as needed to relieve menopausal symptoms.

Additional Information

  • Sunscreens can alter the absorption of oestrogen from Lenzetto. Avoid using sunscreen on the area where you intend to spray Lenzetto, or apply sunscreen at least one hour before using Lenzetto.
  • Use Lenzetto with caution in extreme temperature conditions, such as sauna use or sunbathing.
  • Overweight or obese women may experience reduced absorption of Lenzetto. Consult your doctor if you have concerns.

If You Use More Lenzetto Than You Should

If you use more Lenzetto than prescribed or if a child accidentally uses the medicine, contact your doctor or hospital immediately for advice. Overuse may cause nausea, vomiting, and unusual vaginal bleeding.

If You Forget to Use Lenzetto

If you forget to use Lenzetto, apply the spray as soon as you remember, then continue with your usual routine. If it’s nearly time for your next dose, skip the missed dose and continue as normal. Do not double the dose to make up for a missed one. Missing a dose may increase the likelihood of breakthrough bleeding or spotting.

Stopping Lenzetto

When it’s time to stop using Lenzetto, your doctor will explain how to safely discontinue the treatment. If you have any further questions, consult your doctor or pharmacist.

Who should not use Lenzetto spray?

Medical History and Regular Check-Ups

Using Hormone Replacement Therapy (HRT) comes with risks that need to be considered when deciding whether to start or continue treatment. If you have experienced premature menopause due to ovarian failure or surgery, the risks of using HRT may differ. It is important to discuss this with your doctor. Before beginning or resuming HRT, your doctor will review your medical history and that of your family. A physical examination may also be required, which could include a breast exam or an internal examination if necessary. Once you start using Lenzetto, regular check-ups with your doctor (at least once a year) are essential. During these visits, discuss the benefits and risks of continuing with Lenzetto. Additionally, follow your doctor's advice regarding regular breast screenings.

Do Not Use Lenzetto If:

  • You have or have ever had breast cancer, or if it is suspected.
  • You have cancer that is sensitive to oestrogens, such as cancer of the womb lining (endometrium), or if it is suspected.
  • You have unexplained vaginal bleeding.
  • You have untreated excessive thickening of the womb lining (endometrial hyperplasia).
  • You have or have ever had a blood clot in a vein (thrombosis), such as in the legs (deep vein thrombosis) or lungs (pulmonary embolism).
  • You have a blood clotting disorder (such as protein C, protein S, or antithrombin deficiency).
  • You have or have recently had a disease caused by blood clots in the arteries, such as a heart attack, stroke, or angina.
  • You have or have ever had liver disease, and your liver function tests have not returned to normal.
  • You have a rare inherited blood disorder called "porphyria."
  • You are allergic to estradiol or any other ingredients in this medicine, please refer to the patient information leaflet.
If any of these conditions develop while using Lenzetto, stop using it immediately and consult your doctor.

Warnings and Precautions

Talk to your doctor or pharmacist before starting Lenzetto, especially if you have ever had any of the following conditions, as they may recur or worsen during treatment:
  • Fibroids in your womb.
  • Endometriosis or a history of excessive growth of the womb lining (endometrial hyperplasia).
  • Increased risk of blood clots (see "Blood clots in a vein (thrombosis)").
  • Increased risk of oestrogen-sensitive cancer (e.g., if your mother, sister, or grandmother has had breast cancer).
  • High blood pressure.
  • A liver disorder such as a benign liver tumor.
  • Diabetes.
  • Gallstones.
  • Migraine or severe headaches.
  • Systemic lupus erythematosus (SLE), an immune system disorder.
  • Epilepsy.
  • Asthma.
  • A condition affecting the eardrum and hearing (otosclerosis).
  • Very high levels of fat in your blood (triglycerides).
  • Fluid retention due to heart or kidney problems.
  • Hereditary or acquired angioedema.

Stop Using Lenzetto and See a Doctor Immediately If:

  • You experience any of the conditions mentioned in the "Do Not Use Lenzetto" section.
  • You notice yellowing of your skin or the whites of your eyes (jaundice), which may indicate liver disease.
  • You develop a swollen face, tongue, and/or throat, have difficulty swallowing, or experience hives and difficulty breathing, which may suggest angioedema.
  • You experience a significant rise in blood pressure, with symptoms such as headache, fatigue, or dizziness.
  • You have migraine-like headaches for the first time.
  • You become pregnant.
  • You notice signs of a blood clot, such as:
    • Painful swelling and redness in your legs.
    • Sudden chest pain.
    • Difficulty breathing.
For more information, see the section on "Blood clots in a vein (thrombosis)." Note: Lenzetto is not a contraceptive. If it has been less than 12 months since your last menstrual period, or if you are under 50 years old, you may still need to use additional contraception to prevent pregnancy. HRT and Cancer: FAQs

What is the risk of endometrial cancer with HRT?

Using estrogen-only HRT increases the risk of excessive thickening of the womb lining (endometrial hyperplasia) and endometrial cancer. To mitigate this risk, a progestogen is usually prescribed alongside estrogen for at least 12 days of each 28-day cycle, particularly if you still have your womb. If you have had a hysterectomy, consult your doctor to determine if you can safely use estrogen-only HRT. For women aged 50 to 65 who do not use HRT, about 5 in 1000 are diagnosed with endometrial cancer. In contrast, those who use estrogen-only HRT face a higher risk, with 10 to 60 in 1000 women being diagnosed, depending on the dosage and duration of use.

What should I do if I experience unexpected bleeding while on HRT?

If you experience unexpected bleeding or spotting outside of your monthly withdrawal bleed while using Lenzetto spray, especially if it:
  • Persists beyond the first 6 months,
  • Starts after more than 6 months of use,
  • Continues after you’ve stopped using Lenzetto spray,
You should consult your doctor as soon as possible.

Does HRT increase the risk of breast cancer?

Yes, evidence shows that using combined estrogen-progestogen or estrogen-only HRT increases the risk of breast cancer. This extra risk becomes noticeable within 3 years of use and may persist for 10 years or more after stopping HRT, especially if used for more than 5 years. For example, in women aged 50 to 54 not using HRT, 13 to 17 in 1000 may be diagnosed with breast cancer over a 5-year period. For those on estrogen-only HRT, the risk increases to 16-17 in 1000, and for those on combined HRT, the risk rises to 21 in 1000.

How can I monitor for breast cancer while on HRT?

Regularly check your breasts for any changes such as dimpling of the skin, changes in the nipple, or any lumps you can see or feel. It’s also important to participate in mammography screening programs as advised by your doctor. When having a mammogram, inform the healthcare provider that you are using HRT, as it can increase breast density and affect the mammogram results.

Does HRT increase the risk of ovarian cancer?

There is a slightly increased risk of ovarian cancer with the use of both estrogen-only and combined estrogen-progestogen HRT. For women aged 50 to 54 who are not using HRT, about 2 in 2000 will be diagnosed with ovarian cancer over 5 years. This increases to about 3 in 2000 for women using HRT.

HRT and Circulation: FAQs

How does HRT affect the risk of blood clots?

The risk of blood clots in the veins (thrombosis) is 1.3 to 3 times higher in HRT users compared to non-users, particularly during the first year of use. Blood clots can be serious, especially if they travel to the lungs, potentially causing chest pain, breathlessness, fainting, or even death. In women in their 50s not using HRT, about 4 to 7 in 1000 might develop a blood clot over 5 years. This increases to 9 to 12 in 1000 for women using combined HRT, and to 5 to 8 in 1000 for those using estrogen-only HRT after a hysterectomy.

Does HRT affect heart disease risk?

HRT does not prevent heart attacks. Women over 60 using combined estrogen-progestogen HRT are slightly more likely to develop heart disease compared to non-users. However, for women who have had a hysterectomy and are using estrogen-only therapy, there is no increased risk of heart disease.

Does HRT increase the risk of stroke?

Yes, the risk of stroke is about 1.5 times higher in HRT users compared to non-users. For women in their 50s not using HRT, 8 in 1000 are expected to have a stroke over 5 years. This number increases to 11 in 1000 for those using HRT.

Can Lenzetto spray be transferred to others?

Yes, Lenzetto spray can be accidentally transferred from your skin to others, especially children. To prevent this, avoid letting others touch the treated area until it is dry and cover it with clothing if necessary. If a child comes into contact with the treated area, wash their skin with soap and water immediately. If you notice signs of early puberty (e.g., breast development) in a child who may have been exposed to Lenzetto spray, contact your healthcare provider.

Does HRT affect memory?

HRT does not prevent memory loss, and there is some evidence that starting HRT after age 65 may increase the risk of memory loss. Consult your doctor for advice.

Can Lenzetto spray interact with other medications?

Yes, Lenzetto spray can interact with other medications, leading to irregular bleeding or other effects. Medicines for epilepsy, tuberculosis, HIV infection, and certain herbal products like St John’s wort may interfere with Lenzetto spray. It’s important to inform your doctor about all the medications you are taking.

What should I know about Lenzetto spray and pregnancy?

Lenzetto spray is intended for use in postmenopausal women only. If you become pregnant, stop using Oestrogel and contact your doctor.

What are the side effects with Lenzetto spray?

As with any medication, Lenzetto can cause side effects, although not everyone experiences them. The following conditions are reported more frequently in women using Hormone Replacement Therapy (HRT) compared to those who do not use HRT:

  • Breast cancer

Does Lenzetto spray interact with other medicines?

  • Epilepsy medications (e.g., phenobarbital, phenytoin, carbamazepine)
  • Tuberculosis treatments (e.g., rifampicin, rifabutin)
  • HIV medications (e.g., nevirapine, efavirenz, ritonavir, nelfinavir)
  • Herbal remedies containing St John’s Wort (Hypericum perforatum)

How Lenzetto May Affect Other Medicines

Lenzetto, like other forms of Hormone Replacement Therapy (HRT), can influence how certain medications work:
  • Epilepsy medication (lamotrigine): Using Lenzetto may increase the frequency of seizures.
  • Hepatitis C medications (e.g., combination regimens like ombitasvir/paritaprevir/ritonavir with or without dasabuvir, as well as glecaprevir/pibrentasvir): These may cause an increase in liver enzyme levels (ALT) in women using combined hormonal contraceptives (CHCs) that contain ethinylestradiol. Lenzetto contains estradiol instead of ethinylestradiol, and it is not known whether a similar effect on liver enzymes might occur.
Please inform your doctor or pharmacist if you are taking or have recently taken any other medications, including those obtained without a prescription, herbal remedies, or natural products. Your doctor will provide you with the necessary guidance.
